Athletic Registration Packets

Eligibility to participate on an athletic team for Pacifica Christian High School requires each athlete to provide the Athletic Department with updated information annually. To expedite this process the Pacifica Christian Athletic Department utilizes SportsWare Online, a secure online data entry system. To join the site, specific instructions are provided for both new and existing members. Please provide all applicable information. If this information changes during the year, please log in to update the information. The software allows parents to use the same email address for multiple siblings, but parents must choose a different password for each athlete. This packet must be completed by June 30th.

All students must have a current physical (valid for one calendar year from date of examination) in the Athletics Office. The original copy with a doctor's signature must be submitted to the Athletics Office via uploading it into SportsWare Online or emailing (dropping off) directly to the Athletic Trainer. Only physicals signed by a MD, DO, NP or PA will be accepted as valid.
